Scholarship Application Form
All completed applications for financial aid must be turned in three weeks before the start of each semester to be reviewed by a committee. The committee uses the information given below as a general overview for the evaluation of applications. Other factors may influence the committee’s decision. Application for financial assistance must be made each semester. Only current financial data is considered in determining need.
Income Qualifications
Families who meet the following Federal Income Eligibility Guidelines may apply for needs based scholarship at the Academy of Music. These are the same guidelines used to qualify for free or reduced lunch at school.
In order to determine eligibility, please fill out your household size and maximum household income at the time of the scholarship request. Include gross incomes (before any deductions). For farm and self-employment, use net income after business expenses.

Policies
Qualification Policies
The information you provide may be verified at any time. You may be asked to send additional information to prove your student is eligible to receive this scholarship.
The scholarship is granted only for the current school semester. It will not automatically roll over to the next, nor does one award granted guarantee future awards. A new form must be filled out per family per semester.
Financial assistance will be granted based upon Annual Household Income, total tuition for the current upcoming semester and the availability of scholarship funds.
Efforts are made to distribute financial assistance between as many students as possible. If you know of any individual, business, or other who may want to contribute to these financial assistance funds, please let us know.
Scholarship Policies
- Your account must remain current. Missed payments will void the scholarship after a 10 day grace period.
- Applicants/Families are responsible to pay all tuition fees and other costs that are not covered by a financial assistance award.
- Students must adhere to the Academy of Music attendance policies. This means attending each scheduled lesson, arriving on time, bringing their instruments and other materials to their lessons, and practicing and preparing for each lesson as discussed with the instructor. Failure to abide by these policies may result in disqualification from receiving future awards. Makeup lessons for student schedule changes may not be offered. All scheduled lessons should be taken before the conclusion of the semester.
